The information requested here is data that you should have available at a moment's notice because every organization requires that you provide it. Usually this is all obtained uniformly with an employment application. Regardless of the job level, if you are asked to complete an application, do so.
Be consistent in the details on the application and on the resume. Particular areas are dates and job titles. Be aware of salary information you choose to provide on the application because the question may come up during the interview, and you must provide a consistent answer. Otherwise, you are opening yourself up for additional questions, and your credibility may ultimately be questioned because of a discrepancy.
